  • Did you purchase this device from SMA Medical Supply? Yes / No
  • Device name/model
  • Serial number
  • Description of the problem
You will be provided with a total cost for the repair and will be required to authorize the repair before it takes place. I will contact you with an RMA number and the address of the repair center shortly.
